Does creatine cause hair loss?

If you’ve spent any time reading about creatine, you’ve met this claim. It shows up in gym conversations, in Reddit threads, and in the comment section of every article that doesn’t mention it. Creatine, the story goes, raises DHT, and DHT is what causes male pattern baldness. So creatine makes your hair fall out.

Every part of that chain is worth pulling apart separately, because the first link is much weaker than people assume and the last link was never tested at all.

Where the claim actually comes from

One study. A 2009 trial by van der Merwe and colleagues, run on college rugby players in South Africa. It’s the only piece of evidence anyone cites, and it’s worth knowing exactly what it involved.

The players loaded creatine for seven days, then took a maintenance dose for fourteen. Their DHT rose about 56 percent during loading and settled around 40 percent above baseline afterwards. That sounds dramatic until you look at where it landed: still comfortably inside the normal reference range. Their testosterone didn’t change.

And then the study ended. No hair counts, no follicle imaging, no questionnaire asking whether anyone’s hairline had moved. The researchers never claimed otherwise — the paper is about hormones, not hair. The leap from “DHT went up” to “you will go bald” was made entirely by people reading about the study rather than reading it.

A hormone moving within its normal range for three weeks is not the same thing as losing your hair.

What happened in the fifteen years since

This is the part that gets left out. A finding this alarming, in a field this heavily researched, should have been followed up immediately. It was — and the follow-ups didn’t reproduce it.

Subsequent trials measuring testosterone and its derivatives in creatine users have generally found no meaningful change. A 2021 review pulled together the hormone data across studies and concluded there was no consistent effect on testosterone or DHT. The 2009 result sits alone, unreplicated, in a literature that has otherwise moved on.

That doesn’t make it fraudulent or wrong. Small studies produce outlier results all the time; that’s what small studies do. It makes it what it is: a single measurement in twenty people that nobody has been able to see again.

The link that was never tested

Even if you accept the DHT rise at face value, there’s a gap in the argument nobody addresses.

DHT does drive male pattern baldness — that part is real, and it’s why DHT-blocking drugs work. But baldness depends on whether your follicles are genetically sensitive to DHT, not simply on how much of it is circulating. Men with high DHT and insensitive follicles keep their hair. Men with average DHT and sensitive follicles lose it. The hormone is the trigger; the genetics are the lock.

So the honest version of the claim is: creatine may modestly raise a hormone that, in genetically predisposed men, contributes to hair loss over years. Whether that modest rise is enough to matter has never been measured in a single person.

If you're already losing your hair

Male pattern baldness progresses on its own schedule, and starting creatine around the same time makes for a convincing coincidence. If it’s a real concern for you, the sensible move is a conversation with a doctor about actual treatments — not avoiding a supplement on the strength of one unreplicated hormone reading.

What we’d need to know for certain

A proper answer would take a trial that runs for a year or more, measures hair density directly, and includes men with a family history of baldness. Nothing like that exists. It probably never will, because it would be expensive and slow and the funding for creatine research goes to performance questions instead.

So the accurate position is not “creatine definitely doesn’t cause hair loss.” It’s that after fifteen years, nobody has produced evidence that it does — and the study everyone points to didn’t look.
